Original four page form designed to keep records on liberated internees. This typed form was for internee Josef Sonnentag and contains personal info, how he got to KZ camp and so on. Specific questions were asked such as what color armband he had to wear; Sonnentag answers 'gelbe Streifen' (yellow stripes). We have not seen this form before.
