Description:A horrible reminder of Nazi Racial and Genetic policies. A questionnaire from the "Dienst voor Ras- en Erfgezondheidszorg", literally translated in English "The Racial and Genetic Health Service". During the German occupation of Europe this service ( installed by the German authorities) monitored all hospitalizations of citizens. The forms are printed on thick app. 8.3 x 11 inches paper. The information, that had to be filled in by a doctor attached to the service, is divided in 6 categories. The first 5 are excessive and the 5th part is is totally racial. Asking if the patient belong to the Germanic race or is the patient of "mixed blood" or Jewish?"1 - Personal background: Name, date of birth, address, profession, religion, previous submissions. -2- Status: Married / not married, twins in the family?Children,? 3 - Family Anamnesis: epilepsy's, insanity, psychopathic, tuberculosis, lues, chronic alcoholism, criminal behavior, suicidal, nervous diseases, other points of interest. Father / Mother - Grandparents , Brothers / Sisters - Uncles / Aunts - Diagnosis. 4 - The Patient: Diagnosis, medical history, how was the patient born / were there any difficulties? Education /diplomas? Character? Intellect? Imprisonment? Excessive use of Alcohol and Tobacco? Clinical information?5 - Investigation for Racial elements: Does the patient belong to the Germanic race? If the patient is of "mixed blood" is this Indonesian? Negroid? Jewish?This questionnaire is still blank. The data would have been used by the Nazis to arrest and imprison and kill innocent Jews or other civilians that did not meet their Racial and Genetic policies.
