Partly-printed Document Signed, "Geo. E. Clymer, Captain" 1p. quarto, January 25, 1862, and reads "I certify, on honor, that Samuel G. McNabb a private of Captain George E. Clymer’s Company (‘G’) of the Sixth Regiment of Cavalry Volunteers, of the State of Pennsylvania...having joined the company on its original organization at Reading, Pennsylvania, and enrolled in it at the muster into the service of the United States...is now entitled to a discharge by reason of Certificate of Disability..."
