Johnny Winter's 1950s-era Gibson ES-5.
Several modifications have been made to this guitar, likely by Johnny Winter himself. Originally the ES-5 would have had three P90 single coil pickups with black plastic covers- only middle one still remains. Pair of humbucking pickups with gold plated covers were installed later. The master tone control knob at the treble bout cutaway was replaced by a three-way pickup selector toggle switch, two controls were added (between the treble bout f-hole and edge), and the original third pickup volume control was removed and its hole plugged up.
All four control knobs appear to be original (even though the controls were modified and reconfigured). The trapeze tailpiece is a replacement - the original would have had pointed ends and three raised parallelograms along the top retaining bar. The compensated rosewood bridge, pickguard, and tuners are also original. FON 4942.
