刀通體青白玉質，質地細膩瑩潤，光澤瑩瑩。刀柄立體圓雕為馬首狀，首尾環連。馬眉目深邃，眼圓睜，炯炯有神，鼻翼直挺，張嘴露齒，兩頰豐潤飽滿，雙耳豎立，鬃毛順著頸部披覆兩側，毛髮刻劃一絲不苟，細緻整齊。護手處雕琢為捲曲分向狀，並刻排列整齊的細陰線裝飾。匠人琢磨玉柄時，考量使用的舒適性，於馬頸部淺鑿出手指置放位置。除實用性外，其優雅起伏的線條，更增添整體視覺美感。刃部光素堅韌，刀尖微微上翹。整體造型俐落爽朗，馬首寫實生動，設計繁簡得宜，以玉製作，更彰顯擁有者之尊貴身分。

在眾多刀具中，匕首是一種貼身防衛或暗殺的重要兵器。與長短劍相比，匕首更貼近身體，其製作往往極盡精良，集防身實用性和收藏藝術性於一體。如清宮舊藏，北京故宮博物院庋藏清〈青玉馬頭柄匕首〉（圖一）與本件清〈青白玉馬首柄刀〉，玉柄雕作馬首狀，不僅裝飾精美，更體現北亞民族尚馬傳統。再細觀本品，通體以美玉製作，用料奢華，做工講究，應屬於禮儀裝飾之用，展現清宮武備獨特的藝術魅力。
