Jack Kerouac Life Insurance Policy Settlement Check Stub, With Estate Provenance
Life Insurance Policy Check Stub for Policy Number 41480855, underwritten by John Hancock Mutual Life Insurance Company, Boston, Mass. 7.25" x 3.5". At one time a check was attached to this document as shown in the upper right corner "Attached check is number 406-130173". The document lists "John Louis Kerouac" as the insured. The "Death Benefit of $500.00", the "Additional Benefit of $65.00", the "Settlement Dividend of $5.17", and the premiums due or returned as "$5.17". The full amount of the check that was attached was for $575.34, and the issuing date was "11/19/1969" which was about one month after Kerouac's death.
Provenance: The piece will be accompanied by an estate certification signed by John Shen-Sampas, executor of the Kerouac Estate. John Shen-Sampas is the son of John Sampas, who was the brother-in-law of Jack Kerouac, and the brother of Stella Kerouac, Jack's wife
