Prohibition Era medicinal liquor prescription form issued by the U.S. Treasury Department, Bureau of Internal Revenue, completed in manuscript and dated August 18, 1924. The official government prescription authorized the dispensing of medicinal liquor under the provisions of the National Prohibition Act and bears the signatures of the prescribing physician and dispensing pharmacist, together with cancellation notations, permit numbers, and official filing stamps. Printed on the distinctive orange Treasury Department Prescription Blank for Medicinal Liquor (Form No. 1403, revised February 1922), the document represents an authentic surviving artifact from the early years of Prohibition.

The National Prohibition Act, commonly known as the Volstead Act, prohibited the manufacture and sale of alcoholic beverages in the United States from 1920 to 1933. One of the law’s most notable exceptions permitted licensed physicians to prescribe limited quantities of liquor for medicinal purposes, with pharmacists dispensing the prescriptions on official Treasury Department forms such as this example. These government-issued prescription blanks became an important legal avenue for obtaining alcohol during Prohibition and today serve as fascinating relics documenting one of the most distinctive and controversial periods in American legal and social history.
