Printed for Ferrari's dealer and service network, the form features detailed outline drawings of the 365 GT4 BB overlaid with a reference grid, enabling body damage, accident repairs and defects to be accurately recorded. Spaces are provided for the supplying dealer, chassis number, owner, mileage, repair estimates and warranty status, together with an area for noting faults and the work required. The form offers an interesting insight into Ferrari's factory after-sales procedures during the early 1970s, illustrating the meticulous documentation used to assess and authorise body repairs.

Unused examples are rarely encountered, having generally been completed and discarded during routine workshop use.
