Fred Haise autographed the original paper cover insert from the Apollo 13: 20th Anniversary Edition Blu-ray release, adding the inscription, “Apollo 13 LMP,” in silver ink beneath his signature. The signed insert remains housed in its original Blu-ray case with the Apollo 13 Blu-ray disc included.

Fred Haise (b. 1933) served as the Lunar Module Pilot of Apollo 13, the ill-fated 1970 mission that became one of NASA’s greatest triumphs of ingenuity and teamwork. Although the mission was aborted following the catastrophic explosion of an oxygen tank nearly 200,000 miles from Earth, Haise, Commander James A. Lovell, Jr., and Command Module Pilot John L. “Jack” Swigert successfully returned home after four tense days of emergency procedures and resourceful problem-solving. As Lunar Module Pilot, Haise was responsible for operating Aquarius, the spacecraft that unexpectedly became the crew’s lifeboat following the accident. His role in conserving power, navigating the crippled spacecraft, and executing the return to Earth cemented his place in one of the most celebrated chapters in the history of human spaceflight. The events of Apollo 13 were immortalized in the acclaimed 1995 film Apollo 13, bringing the extraordinary mission and Haise’s courage to a new generation.
