Frank Sturgis original Police Department–issued taxi driver license, issued under his legal name Frank A. Fiorini, measuring approximately 4.25 x 7 inches. The license was issued by the Police Department, County of Norfolk, dated June 1, 1950, and includes an affixed original photograph of Fiorini, secured with period staples. The permit issue number is 378, with the license set to expire December 31, 1950.
The document lists Fiorini’s address as 326 Bank Street, Norfolk, Virginia, and notes that the permit was issued pursuant to an ordinance adopted December 11, 1945. The license bears the printed name of F. Perry Williams, Major Superintendent, N.C.P.D., along with an original green-ink authorization signature from "D.H. Barkley." Standard warnings against bending or folding are printed on the face.
The verso is titled “Arrest Record” and contains ruled lines for entries; no charges or dispositions are filled in. Period staple remnants are present, and the printed certification text states that removal or alteration of the photograph renders the license void, underscoring its function as an official working credential.

Frank Sturgis (1924–1993) was a former CIA operative and anti-Castro paramilitary who became infamous as one of the five Watergate burglars arrested in 1972 at the Democratic National Committee headquarters. Beyond Watergate, Sturgis has long been a subject of JFK assassination conspiracy theories due to his intelligence connections and murky activities in Cuba. Some theorists, including New Orleans District Attorney Jim Garrison, suspected him of involvement or knowledge relating to the assassination of President John F. Kennedy in 1963. Though no official evidence ever linked him directly to the crime, his association with key figures and covert operations has kept his name prominent in speculative accounts of the event.
